Substance Use Disorder Oper The following positions within Inpatient Adolescent Psychiatry, Inpatient Adult Psychiatry, Psychiatric Emergency Services, CARES, and Outpatient Behavioral Health Services are offering the opportunity to enroll in a new program offering a cash incentive to recognize your work treating patients with substance use disorders. This program is sponsored through the ACCESS (Advancing Careers, Competencies, and Equity in Substance Treatment Services) grant. Employees must be >0.6 FTE to be eligible. 50% of the bonus will be received 90 days after signing the program contract.

* Registered Nurses (RNs) may earn up to $7,500 by December 2024
* Behavioral Health Technicians (BHTs) may earn up to $3,500 by December 2024
* Care Navigators may earn up to $3,500 by December 2024 Job Summary

Under general professional supervision, performs behavioral health case coordination services including planning, organizing, and treatment referral/disposition. Delivers services in accordance with the approved plan of treatment and plan of care. Assists behavioral healthcare providers in understanding significant social and emotional factors related to the patient's behavioral health issues.

Essential Functions:
  • Identifies client's behavioral health needs, plans for implementation and/or coordinates care plans through resource acquisition, monitoring, advocacy, collaboration, and evaluation. (20%)
  • Coordinates follow-up treatment referrals to various levels of care including outpatient therapy and psychiatry, intensive outpatient programing, long term care and other services as indicated. (10%)
  • Evaluates emotional components, impact of health issues and assists client to identify disruptive factors to establish treatment goals. Adheres to and complies with rules and regulations of the City, Agency, Department and Service. Practices in accordance with ethical principles established in NASW code of ethics and by Denver Health. (10%)
  • Assists clients to maximize adjustment to behavioral health diagnosis/illness/disability/trauma by addressing biopsychosocial issues and enhancing problem solving and coping skills. (10%)
  • Documented activity reflects patient involvement in goal setting with written information consent documented. Completes documentation of care coordination activities and interventions per established standards and guidelines. Prepares and submits data and information to behavioral health supervisor in a timely manner. (10%)
  • Develops knowledge base of risk factors for psychosocial functioning, wellness and health promotion and provides basic education to patient. (10%)
  • Utilizes methods, techniques and approaches to the delivery of care that are consistent with the patient population and in conformance with the specific care plan developed for each patient. (10%)
  • Assists client in achieving treatment goals while applying basic knowledge of individual, group and family dynamics and treatment methods. (10%)
  • Monitors and evaluates effectiveness of services to consumers and modifies interventions plan as indicated. (10%)

Education:
  • Bachelor's Degree Psychology, Social Work, Human Services or other closely related health field Required

Work Experience:
  • 1-3 years 1 year experience required Required

Licenses:

    Knowledge, Skills and Abilities:

    • Knowledge of emotional and social factors that affect patient's health.
    • Excellent interpersonal skills.
    • Skilled in identifying problems/opportunities to improve care.
    • Ability to utilize resources, such as family and community agencies to assist the patient to improve life in the community.
    • Ability to help the patient and family to understand, accept and follow healthcare recommendations.
    • Ability to collect key information from the patient, family and the community as part of the care coordination assessment.
    • Ability to communicate to the appropriate staff information about the patient's care plan.
    • Ability to manage time sufficiently to achieve established goals.
    • Proficiency with software applications such as Microsoft Word, Excel, and Outlook.
    • Proficiency in operating Electronic Medical Records, including EPIC.
