Responsibilities

We are hiring for our Parks Area Internal Medicine Clinic, where you'll provide vital support to patients by addressing their emotional, social, and psychological needs. p style="margin: 0px;"> In this role, you'll collaborate with a multidisciplinary team to improve patient outcomes, offering counseling, resource coordination, and advocacy. - Provide individual and group counseling services to patients dealing with chronic illness, mental health concerns, or social challenges.
- Assess patients' psychosocial needs, develop individualized care plans, and provide interventions as needed.
- Collaborate with physicians, nurses, and other healthcare providers to ensure a holistic approach to patient care.
- Assist patients in accessing community resources, including housing, financial assistance, mental health services, and support groups.
- Provide crisis intervention and emotional support for patients and families during difficult medical diagnoses or treatment decisions.
- Educate patients and families on coping strategies, mental health care, and stress management.
- Document patient interactions, assessments, and care plans in the electronic medical record (EMR) system.
- Facilitate referrals to behavioral health services, social services, or other relevant healthcare providers as needed.
- Participate in multidisciplinary team meetings and contribute to the overall care planning process.
- Stay current with best practices in mental health, social work, and community resources.
Qualifications
- Active LISW or LMHC license in Iowa, Independent.
- Masters Degree in Social Work.
- Prior work experience (2 yrs), preferably in an outpatient or ambulatory care setting.
- Strong knowledge of mental health issues, chronic illness management, and community resources.
- Excellent communication, counseling, and crisis intervention skills.
- Ability to work collaboratively in a multidisciplinary healthcare team.
- Experience working with diverse patient populations and understanding their unique needs.
- Proficiency with electronic medical records (EMR) and documentation.
- Commitment to patient confidentiality and ethical standards in social work.
