Job Summary
Performs comprehensive health assessments for stable long-term patients. Assesses and manages care for acute, minor, episodic illnesses and injuries. Interviews and advises patient/families regarding health/illness prevention. Performs follow up exams. Provides assessment, evaluation, and management of clients with chronic stable illnesses. Collaborates with client and family to determine physiologic, emotional, and environmental needs and priorities. Provides case management services to assist clients and families with the arrangement for, or refer patients to needed services. Manages client/family health concerns through telephone triage and consultation as needed. Provides services, which may include the ordering of appropriate diagnostic and laboratory tests, the prescription of pharmacological agents, and other nonpharmacological treatment modalities. Maintains and reviews patient records, charts, and other pertinent information. Performs patient callbacks, reports diagnostic findings and provides consultation. Participates with physicians in review of patient's health records. Participates in call coverage.
Experience Qualifications
* 1-3 years Candidate must demonstrate a minimum of two years of clinical experience as a licensed APNP with at least one year of continuous performance in an urgent care or emergency room setting. Required

Licenses and Certifications
* Current licensure as RN in State of WI. Upon Hire Required
* Valid as APNP in the State of WI. Upon Hire Required
* DEA certification Upon Hire Required
* Current certification in BLS, ACLS, and PALS with certification occurring every two years. within 1 Year Required
