LOCATION: Sierra Vista Corps

STATUS: PT (20hrs/wk)

QUALIFICATIONS:



  • Bachelor's degree in related field required (relatable experience may substitute on a 2:1 ratio for educational requirement)
  • Bi-lingual (Spanish) preferred
  • Ability to use social service software program
  • Ability to access at risk participants and situations
  • Proficiency in Microsoft Office Software (Word, Excel, etc.)
  • Strong verbal and written communication skills
  • Excellent organizational and file management skills
  • Ability to maintain strict confidentiality
  • Ability to work in a team setting
  • Must be able to prioritize responsibilities and manage time efficiently
  • Ability to work with people of diverse backgrounds and circumstances
  • Must pass a criminal background check


RESPONSIBILITIES:



  • Conduct interview, assess immediate & long-term needs, and develop case plans
  • Maintain a updated knowledge base of criteria for food, rent, & utility assistance & the processes to enroll participants
  • Provide case management and needed services to low-income and/or at-risk individuals/families in compliance with program policies and procedures
  • Prepare and submit required reports in a timely manner
  • Maintain accurate program records of EFSP, SHARE AND REACH programs, as well as volunteers and vendors
  • Keep statistics as related to demographics, types of assistance given
  • Write accurate and concise case notes for each client
  • Ability to mentor clients related to budgeting, banking and other basic life skills
  • Provide assistance with seasonal events and drives as needed
  • Maintain neat and organized work area
  • Network with other agencies and organizations
  • Other duties, as assigned as they relate to the position of Case Manager.


PHYSICAL REQUIREMENTS:

The employee must regularly lift and/or move up to 20 pounds, frequently lift and/or move up to 40 pounds, and occasionally lift and/or move up to 60 pounds. Specific vision abilities required by this job include close vision, distance vision, color vision, depth perception and ability to focus. The employee must have the ability to operate a telephone, desktop or laptop computer, as well as the ability to access and produce information from a computer, and to understand written information.
