The State Of Vermont Department of Disabilities, Aging and Independent Living (DAIL) is looking for a Senior Planner to work on the Money Follows the Person (MFP) grant.In 2011, DAIL was awarded a five year $17.9 million "Money Follows the Person" (MFP) grant from the Centers for Medicare and Medicaid Services (CMS) to help people living in nursing facilities overcome barriers to moving to their preferred community-based setting. The grant works within the Choices for Care program and provides participants the assistance of a Transition Coordinator and up to $2,500 to address barriers to transition.

In this role, you will:

  • Design and implement management information systems related to long-term care services and the MFP grant.
  • Assist the Program Director in managing all required internal, state and federal reporting, tracking and data management, including management of MFP benchmarks and data integrity.
  • Perform data analysis.
  • Interpret/translate program reporting needs into data elements.
  • Complete required MFP reports as expected.
  • Assist in the development of the annual budget.
  • Assist MFP Quality Management Specialists to manage program tracking systems.
  • Assist in identifying MFP website needs and developing a plan for updating the Department's website.
  • Oversee Quality of Life surveys and data submission.
  • Oversee Critical Incident Reporting systems and data submission.
  • Perform all required Quality Management and program Utilization Review activities.
  • Assist with independent evaluation for MFP as necessary.
  • Assist in writing/updating the MFP Operational Protocol as needed.
  • Assist the Program Director with financial and performance reporting as needed by doing research, statistical and program evaluation.
  • Provide on-going monitoring of all program activities.
  • Supervise staff in the absence of the Project Director.

This position is located at the DAIL offices in Waterbury, with the opportunity for telework. This limited-service position has been continually grant funded by CMS since 2012.

Environmental Factors

Duties are performed in a standard office setting.

Minimum Qualifications

Master's degree in statistics, computer sciences, human service, or social sciences discipline, AND one (1) year or more of experience with scripting or programming languages, or database management in a professional or educational setting.

OR

Bachelor's degree in statistics, computer science, human services, or social sciences discipline, AND BOTH one (1) year or more of in research, statistics, or program evaluation where the activities include gathering, analyzing, and interpreting statistical data AND one (1) year or more experience with scripting, or program languages or database management in a professional or higher educational setting.

OR

Four (4) years experience in research, statistics, or program evaluation where the activities include gathering, analyzing and interpreting statistical date AND Four (4) years or more of experience with scripting or programming languages or database management.
