DESCRIPTION OF WORK

As an Aging Care Manager 2 for the Care Management Unit, you will conduct home visits to initiate care plans and educate consumers on available resources in the community in addition to those available through AAA programs. You will also visit participants homes to respond to emergency situations, monitor services as specified in the care plan, provide emotional support to consumers, and assist with resolving any problems or issues that may arise. This professional will arrange for financial management services for consumers who are unable to independently tend to their finances. This may include establishing budgets with consumer participation or arranging for an agency to act as a representative payee for a consumers public benefits. Other duties include, but are not limited to, assisting consumers in negotiating the aging service system or assisting consumers with pre-placement and placement activities when community services are no longer sufficient to maintain consumers in the community. Work Schedule and Additional Information:
- Full-time employment
- Work hours are 8:00 AM to 4:00 PM, Monday - Friday, with a 30-minute lunch.
- Salary: In some cases, the starting salary may be non-negotiable.

REQUIRED EXPERIENCE, TRAINING & ELIGIBILITY

QUALIFICATIONS Minimum Experience and Training Requirements:
- Six months as an Aging Care Manager 1 or a County Caseworker 1; or
- Successful completion of the County Social Casework Intern program; or
- Six months of professional human services or nursing experience; and a bachelor's degree in social sciences, behavioral sciences, human services, or a closely related field; or
- An equivalent combination of experience and training.

- Hiring preference for this vacancy may be given to candidates who live within Carbon, Columbia, Lackawanna, Luzerne, Monroe, Schuylkill, Sullivan, or Wyoming counties. If no eligible candidates who live within these counties apply for this position, candidates who reside in other counties may be considered.
- You must be able to perform essential job functions.
