DESCRIPTION OF WORK

As an Aging Care Manager 1, you will be required to complete the PA Individualized Assessment System (SAMS) trainings to obtain access to the state database where journal notes and care plans are entered for consumers. You will be responsible for providing care management for a caseload of aging consumers as assigned by the Aging Care Management Supervisor or Aging Deputy Administrator. Your caseload may increase or decrease with changes in the aging population's need for services. Work includes developing care plans through an evaluation of the consumers' social contacts and supports, their activities of daily living, status of their cognitive functioning, their physical health, and their economic and other resources.

Upon completion of training, you will be expected to exercise initiative and independent judgment in discharging assignments as well as seeking advice and help only with the more unusual and difficult cases. Your work will be reviewed by a professional or administrative superior through an evaluation of case records, individual and group conferences, and an evaluation of reports. After successful completion of a six-month probationary period, Aging Care Manager 1s will be promoted to an Aging Care Manager 2, per union agreement.

Work Schedule and Additional Information:

  • Full-time employment
  • Work hours are 8:30 AM to 4:30 PM, Monday - Friday, with a 30-minute lunch.
  • On-call hours and overtime will be required.
  • Salary: In some cases, the starting salary may be non-negotiable.

REQUIRED EXPERIENCE, TRAINING & ELIGIBILITY

QUALIFICATIONS

Minimum Experience and Training Requirements:

  • Three years as an Aging Case Aide 2; or
  • A bachelor's degree which includes or is supplemented by twelve (12) credits in social sciences, behavioral sciences, human services, or a closely related field; or
  • An equivalent combination of experience and training.
  • Applicants will be considered to have met the educational requirements once they are within 3 months of graduating with a qualifying degree.

  • Hiring preference for this vacancy may be given to candidates who live within Potter County. If no eligible candidates who live within Potter County apply for this position, candidates who reside in Cameron, Clinton, Elk, Lycoming, McKean, or Tioga counties will be considered. If no eligible candidates who live within these counties apply for this position, candidates who reside in other counties may be considered.
  • You must be able to perform essential job functions.

  • You must pass a background investigation.
  • This position falls under the provisions of the Older Adult Protective Services Act.
    • Under the Act, a conditional offer of employment will require submission and approval of satisfactory criminal history reports, including but not limited to, PA State Police and FBI clearance.
  • This position falls under the provisions of the Child Protective Services Law.
    • Under the Law, a conditional offer of employment will require submission and approval of satisfactory criminal history reports including, but not limited to, PA State Police clearance, PA Child Abuse history clearance, and FBI Fingerprint clearance.
