Job Summary Provides primary care and or advanced life support, including medical evaluation, treatment, and stabilization of the critically ill and injured with the goal of reducing morbidity. Supports existing health services; provides mobile integrated health services in partnership with other health professionals; extends access to health services delivery in underserved and general populations, including, but not limited to, primary care, public health, disease management, prevention and wellness, and mental health. Exercises appropriate judgement in all situations. Strives to ensure prompt response to emergency calls when required.
Essential Functions

Provides acute and chronic medical care, health assessments, disease monitoring, patient education, and medication compliance to patients enrolled in the community paramedic program to improve health and reduce emergency department usage. Administers patient care consistent with departmental and Community Paramedic protocols and physician orders. Performs all primary job responsibilities required for a Paramedic and responds to emergent calls when requested. Examines, screens, treats, and coordinates health services for patients. Interacts with primary care services, schedules transportation to and from appointments, and assists with health logistics to ensure patient access to necessary care. Conducts post-hospital discharge follow-up care including, but not limited to, monitoring medication, dressing changes and checking vital signs to improve health and reduce readmission. Observes, records, and reports to physician, patient's conditions and reactions to drugs, treatments and significant incidents. Coordinates appointments and follow-up with physicians and hospitals. Communicates and interacts with the interdisciplinary care team. Conducts patient education including, but not limited to, diabetes prevention / treatment, hypertension, congestive heart failure (CHF), chronic obstructive pulmonary disease (COPD), falls assessment, injury evaluation, geriatric frailty and nutrition. Attends required training to maintain current certification and licenses. Attends all meetings as requested and required. Completes required reports and records; monitors and accounts for all Community Paramedic (CP) vehicle supplies / equipment; ensures that CP vehicle supplies, and equipment are in safe and proper working condition and meet "readiness" standards; restocks CP vehicle as needed; completes station duties as needed. Ensures that assigned CP vehicle complies with departmental "readiness" standards. Develops and completes appropriate reports and templates for the Community Paramedic program. Performs POC testing with approval of Administration Performs other duties as assigned.
Supervisory/Management Responsibilities This is a non-management job that will report to a supervisor, manager, director or executive. Minimum Requirements
Education - High School diploma or equivalent OR post-high school diploma/highest degree earned. Approved Paramedic Education Program. Associate's degree in Paramedicine related field of study preferred. Experience - Five (5) years paramedic experience.
In Lieu Of
In Lieu of the education and experience requirements noted above, completion of an approved Paramedic training program and two (2) years functioning as a Community Paramedic may be considered. A valid driver's license, from a state other than South Carolina, may be accepted if primary residence is not SC. The required active South Carolina AND National Registry EMS certification meets the educational requirements for this role.
Required Certifications, Registrations, Licenses
SC PARAMEDIC - AEMT SCDL - SC DRIVERS LICENSE: Valid driver's license and an acceptable five-year motor vehicle record as defined by the Acceptable Motor Vehicle Record (MVR) Chart Community Paramedic Certification (must be enrolled in a SC approved CP Training Program and/or obtain Certification within six (6) months of hire).
