Job Summary and Responsibilities

Acentra seeks Care Coordinators to join our growing team.

Job Summary:

Provides field-based care coordination outreach activities to support care management program delivery to identified beneficiaries to develop, implement, and monitor person centered service plans. Collaborate with beneficiaries and their identified treatment team to ensure needed services are delivered appropriately and address any barriers to access.

*Positions are hybrid within the State of Arkansas. We are hiring one Care Coordinator in each of the following regions: Northeast AR, Northwest AR, North Central AR, and Central AR.

Job Responsibilities:

  • Ensures the responsible delivery of comprehensive services to beneficiaries enrolled in care coordination
  • Conducts initial assessments of services of each assigned beneficiary
  • Develop and monitor Person Centered Service Plans (PCSP) for each beneficiary on assigned caseload
  • Engage and collaborate with home and community-based providers, caregivers, and community resource providers to contribute to PCSP as needed
  • Facilitate and participate in annual treatment team meetings/reviews to develop and review PCSP
  • Maintain at least monthly phone contact and quarterly in person/face to face engagement with assigned beneficiary/legal guardians to monitor progress with PSCP and service delivery
  • Document all care coordination activities within contract timeframes
  • Regular travel to treatment team meetings, and beneficiary visits
  • Maintain a secure home office environment
  • Adhere with all relevant Care Coordination and Acentra Health policies and procedures as well as contractual requirements

Qualifications

Required Qualifications/Experience:

  • Bachelor's degree in social science or health-related field
  • Minimum of two years current experience in care coordination (field environment preferred)
  • Minimum of one year of recent experience in clinical environment, preferably in field delivery, working with individuals with developmental or intellectual disabilities or with behavioral health beneficiaries

Preferred Qualifications/Experience:

  • Experience working with adults with behavioral health and/or Intellectual disabilities
  • Public sector experience: Medicaid
  • Knowledge of principles, ethics, and precepts of care management service delivery
  • Knowledge of customer service principles
  • Knowledge of medical terminology
  • Knowledge of regulatory and accreditation standards
  • Strong field and telephonic interviewing skills
  • Strong computer skills, e.g. care management applications, Internet/Web, Microsoft Office (Word, Excel, Access)
  • Ability to demonstrate purposeful and outcomes directed critical thinking within daily operational activities
  • Ability to receive verbal and written feedback in a professional manner and implement performance and productivity improvements as needed
  • Ability to interpret real-time and historical information to inform delivery on overall accountabilities

Compensation

The pay range for this position is $17.33-29.80 per hour.
