Organizational Unit Overview |
The Department of Pediatrics is one of the largest clinical departments within the East Carolina University Brody School of Medicine. The mission of the Department is to provide quality patient care, to teach medical students and residents, and to do research. It consists of 16 sections, Pediatric Outpatient Center, and laboratories (research and clinical). The sections of the Department are: Administration, Adolescent Medicine, Cardiology, Children's Advocacy Center, Children's Developmental Services Agency, Critical Care and Pulmonology, Endocrinology/Diabetes, Family Autism Center, Gastroenterology, General Pediatrics, Healthy Weight, Hematology/Oncology, Infectious Disease and Immunology, Medicine-Pediatrics, Neonatology, Nephrology, Neurology, and Sleep Medicine. The Department is funded from State appropriated monies, practice plan generated income and by grants awarded. It is the mission of the Department of Pediatrics, East Carolina University Brody School of Medicine, to provide exemplary medical education/training of physicians in pediatric medicine at the undergraduate and graduate levels. Additionally, it is the mission of the department to provide model comprehensive primary and consultative subspecialty care to children of eastern North Carolina, to advance scientific knowledge through basic and clinical research, and to provide continuing medical education opportunities for physicians in the region. The achievement of this mission requires a full faculty commitment to the goals and operations of the Department of Pediatrics, the East Carolina University Brody School of Medicine, and East Carolina University. |
Job Duties |
The Division of Pediatric Hematology/Oncology is recruiting for a Transition Coordinator. The major purpose of this position is to coordinate services related to transitioned aged patients (Bleeding and Clotting Disorder, Sickle Cell, Oncology, and other Hematology) and education for patients and their families, as well as extended care givers. The overseeing and coordination of these services includes but is not limited to:
- Conduct biopsychosocial assessments on all transition aged patients.
- Conduct Transition Readiness assessments on transition aged patients (age 18-25) which includes assessing stress thresholds, understanding of chronic disease, disease management knowledge, and case management needs. This assessment is done for all incoming patients and must be repeated on an annual basis.
- Based on assessments, make necessary learning objective plans with each patient as needed, treatment recommendations, referral to therapy and/or other social services and community-based organizations.
- Act as a liaison between transition aged patients and the adult bleeding disorder/sickle cell medical team as needed.
- Provide supportive counseling for transition aged patients when appropriate. Work alongside comprehensive team members to assist transition aged patients in orienting to the adult sickle cell clinic, locations, processes, and care team when needed.
- Work as a member of the comprehensive care team to collaborate with providers and patient navigators in ensuring improved patient care and outcomes.
- Work in a hospital setting, or other designated settings, to ensure that the patients' needs are properly identified. This will include, but not limited to, hospital visits, ED visits, other medical appointments, and all other approved locations to work efficiently with the patient to provide stability.
- Work with interdisciplinary teams while patient is hospitalized to prepare for discharge and ensure medical follow-up is secured and provided for the patient's needs.
- Works with patients to empower them to become an active participant in their health care.
- Documents and is competent in listening, sharing information 1:1 or in larger groups using established written and oral communication systems.
- Responsible for updating patient notebooks through the coordination of the HTC Coordinator and lead Social Worker. Patient notebooks include, but are not limited to; Bleeding Plans, Sickle Cell Disease, vWD, Sickle Cell Fever Cards, Hematology Fever Cards.
- Maintains internal and external databases for all Peds Hem/Onc patient populations. Databases are updated daily, weekly, monthly, and quarterly utilizing EPIC and sign in sheets. Databases include, but are not limited to; Hem/Onc Patient Database, NC Sickle Cell Database, ACCEL Grant Registry, Clinical Manager/ATHN, Hemophilia Treatment Center (HTC) Database.
- Assist where needed in coordination and development of grant related projects. Collaborate with Division's Administrator, Grant PI, and related staff, to work on grant reports during the mid-year and final report time periods. Grants include, but are not limited to; Maternal and Child Health Bureau Grant, Department of Health and Human Services Sickle Cell Grant.
- Assist the division chief in searching and applying for potential grants that will benefit our transitioning aged patient populations.
- Assist in the development of Division's communication plan for patient education material and patient events that are distributed, via printouts/mailings/emails, to patients, caregivers, community partners, and healthcare professionals.
- Work with division administrator to establish effective communication plans with related patient educational events/materials.
- Accepts alternate assignments as required, by supervisor or division chief.
Currently, hours of clinic operation are from 8:00 a.m. to 5:00 p.m., with a flexible scheduled late day to accommodate patient care, Monday through Friday.

Minimum Education/Experience |
Associate degree in a related Human Services discipline and two years of experience related to the area of assignment, or equivalent combination of training and experience. All degrees must be received from appropriately accredited institutions. |
