Ensures appropriate delivery of psychiatric services for clients in the Behavioral Health Services from admission through discharge by providing: individual, family and group therapy; case management to include development of a Master Treatment Plan and Discharge Plan; diagnosis, assessment and evaluation. Serves as a member of the intra-disciplinary treatment team and provides direct delivery of services for assigned patients. Provides individual and group therapy services.
MINIMUM QUALIFICATIONS: EDUCATION, CERTIFICATION, AND/OR LICENSURE: 1. Bachelor's Degree in Social Work, Counseling or Clinical Psychology and currently enrolled in Master's program for degree's listed. 2. CPR Certification required within 30 days of hire 3. Crisis Prevention Institute (CPI) certification required within 30 days of hire 4. West Virginia state and Federal criminal background check required for DHHR WV CARES regulated area. CORE DUTIES AND RESPONSIBILITIES: The statements described here are intended to describe the general nature of work being performed by people assigned to this position. They are not intended to be constructed as an all-inclusive list of all responsibilities and duties. Other duties may be assigned. 1. Responds to crisis situations as per policy and procedures. 2. Develops and ensures implementation of a family education plan. 3. Provides assessments and referral services to the Behavioral Health Connections as assigned by Supervisor. 4. Serves as a member of the intra-disciplinary team as evidenced by treatment team meetings and record documentation; will collaborate with the psychiatrist as part of the patient's overall treatment. 5. Understands the process of Advance Directives and acts upon as per patient's best interest. 6. Assists the charge nurse in the process of ensuring the safety of patients/families/staff. (safety checks, assessment of escalating behavior, suicidal potential) 7. Ensures that patient rights and responsibilities are enforced at all times. 8. Performs UR duties as assigned by Supervisor and completes documentation of medical records as per policy and procedure 100% of the time. 9. Coordinate and ensure delivery of service for assigned patient as evidenced by chart reviews, observation, and feedback from physicians. (case management). 10. Coordinates treatment team meetings for assigned patients as evidenced by chart reviews, observation, and feedback from the team members. 11. Completes the initial interdisciplinary assessment of patient to include psychosocial history and discharge information to formulate plan of care as evidenced by chart reviews, observation, and feedback. 12. Develop, implement, evaluate, and revise treatment plan for assigned patients following documentation, observation, and treatment team feedback. 13. Provides case management duties for assigned patients. 14. Recognize and manage crisis situations with patient condition keeping self as a role model. 15. Conducts individual/group/family therapy for assigned patients as per policy and procedure. 16. Provides therapy services to geriatric patients as evidenced by age appropriate interventions. 17. Provides therapy services to adult psychiatric and chemical dependency patients by tailoring techniques that address the specific age population needs. 18. Conducts in-service for other staff to advise them as to the needs of the population served. 19. Assists Supervisor in developing specific program processes to address the needs of the population served. 20. Serves as a role model as it relates to communication and therapeutic relationship between patients and staff. 21. Provides aftercare services as assigned by policies and procedures. Knowledge of DSM 5 as evidenced by appropriate diagnostic impressions and symptoms of mental illness. 2. Demonstrates knowledge of CPI and the management of aggressive patients. 3. Exhibits knowledge of community resources in assisting patients to connect with the community and to receive support from community agencies. 4. Able to identify a discharge plan at the time of admission and through out the treatment of each patient assigned to him/her (as evidenced by documentation). 5. Demonstrates skill in developing an individualized treatment plan with objective, measurable, time limited goals and interventions. 6. Demonstrates skill in assessment and re-assessment of patients as evidenced by documentation from the time of admission to discharge. 7. Demonstrates knowledge of the addiction process as evidenced by treatment plan interventions. 8. Demonstrates skills in identification of abuse situations and follows the process of reporting as per policy and procedure. 9. Demonstrates skills in the assessment of pain and the implications of pain management and addiction. 10. Demonstrates ability to work effectively with clients experiencing emotional disturbance/behavior problems and/or chemical dependency. 11. Demonstrates skill in developing an individualized treatment plan with objective, measurable, time limited goals and objectives. 12. Demonstrates skill in assessment and re-assessment of patients as evidenced by documentation from the time of admission to discharge. 13. Demonstrate knowledge of the addiction process as evidenced by treatment plan interventions. 14. Able to implement appropriate treatment interventions so that the patient can be maintained at the least restrictive environment.
