  1. Oversee and manage all members of the HIV Case Management team which includes management of time and attendance, conducting performance evaluations, and being involved in disciplinary procedures with the assistance of the Director of Care Coordination Programs.
  2. Schedule, design, and conduct department meetings. This includes organizing and tracking any mandatory training that needs to be completed.
  3. Organize the hiring, training, and onboarding of all new hires.
  4. Participate in Quality Management by analyzing clinical and case management data reports periodically to inform and lead the implementation of CQI projects in the Ryan White (RW) Program.
  5. Conduct periodic chart audits and work with the RW case management team to ensure continuity of care for patients by complying with documentation of eligibility and other program requirements in the EMR accordingly. This may include annual eligibility and enrollment, RW program required assessments, case management service plans, frequency of medical appointments, labs, screenings, treatments, and referrals.
  6. In coordination with the Director of Care Coordination Programs, lead the development, implementation, and evaluation of current and new projects within the RW program. Facilitate multidisciplinary meetings and lead department committees.
  7. Under the Director of Care Coordination Programs participate in preparing grant applications and reports.
  8. Represent the department and actively participate in internal interdepartmental or agency-wide and external meetings, committees, training, and projects as requested by the Director of Care Coordination Programs.
  9. Lead the facilitation of the RW Consumer Advisory Board (CAB) to present information about current services and quality projects to RW patients and obtain their input to inform the development and implementation processes of those new projects and services.
  10. Annually facilitate enrollment of patients into SSCHC's HIV Program, and review documentation to determine eligibility to receive services under the Ryan White program.
  11. Complete financial verification including sliding fee scale assessments on all program participants and educate patients about their rights and responsibilities related to the sliding fee scale, and benefits of participation in the RW program including a cap on charges, access to specialty care, oral health care, laboratory, imaging and/or pharmacy.
  12. Submit payment requests to the Director of Care Coordination Programs for emergency financial assistance, specialty, or oral health bills of patients eligible under the RW program.
  13. Meet with patients to resolve billing disputes and work to link them to financial counseling in-house or through external referral when applicable.
  14. Work closely with other departments in charge of patient registration, billing, and patient navigators of the clinic to troubleshoot barriers to accessing those services for RW patients.
  15. Other responsibilities as assigned.


Qualifications:

  1. Bilingual (Spanish/English) is strongly preferred.
  2. Bachelor's degree in business administration or public health-related field preferred; consideration will be made for a suitable combination of previous academic volunteer and professional experience.
  3. Background working with grants, data collection, and report generation.
  4. Competency in Microsoft Word, Excel, and PowerPoint.
  5. Exceptional organizational skills.
  6. Excellent oral and written communication skills.
  7. Familiarity with medical terminology.
  8. Ability to relate well to people from diverse socioeconomic and cultural backgrounds including sexual orientation and gender identity.
  9. Valid Wisconsin driver's license, use of an auto, and current auto insurance.
