Utilizes skills to retain members in the assigned provider practices and supports new membership after the initial enrollment period. Coordinates in office wellness events as a result of interventions management to improve gap closures in the field. Coordinates with Health Education and contacts members telephonically to schedule them for appointments. Participates in the Member Advisory and Provider Advisory Meetings as needed. Maintains superior member/provider relationships and ensures the highest quality of care coordination.One-year experience in a member and/or provider engagement role preferred Ability to travel up to 80% of the time is mandatory Qualifications: ALD - Associate's Level Degree, HS - High School Grad or Equivalent (Required) Customer Service, Managed CareSkills Active Listening, Communication, Critical Thinking, Microsoft Office, Monitoring, Service Orientation, Social Perceptiveness, TroubleshootingSentara Healthcare prides itself on the diversity and inclusiveness of its close to an almost 30,000-member workforce.
