Full-time Description
Case Manager II People Incorporated is seeking a Case Manager II to join our Targeted Case Management Dakota County team to work with clients who are diagnosed with chronic mental illness. We support people in connecting with resources to live as independently as possible. We help them develop self-awareness to make positive plans for the future through frequent meetings in the community. Case Managers develop long term relationships with their clients and operate autonomously, creating their own schedules. Hours: Full - Time, Monday - Friday, business hours Location: Dakota County, Minnesota. Hybrid Range: $24.30 - $25.35 Hourly, based on qualifications Hiring Bonus: $1,000 to external candidates People Incorporated Mental Health Services is the largest nonprofit mental health provider in Minnesota.
