This position is 100% onsite at the Martha Eliot Health Center location in Jamaica Plain.
Part time, 20 hours per week.

The Healthy in the City (HITC) Program Coordinator will be responsible for:

  • Meeting with families, youth, and care teams. Assessing the level of youth and family engagement, barriers to care, adherence, and food resources. Working with the Nutritionist to develop food strategies.
  • Recruiting youth and families into the program and taking a proactive approach to maximize youth and family engagement in the program.
  • Developing and providing physical activity (e.g., exercise groups, dance classes) and nutritional (e.g., cooking classes, grocery store trips) programming for enrolled youth.
  • Maintaining active relationships with clinicians and other providers in the health center to promote collaboration, increase program visibility, and augment participation from patient families.
  • Collaborating closely with other obesity-focused programs in the health center.
  • Making post-visit follow-up appointments and calls to assess progress, obtain qualitative and quantifiable data concerning proposed healthy initiatives implemented for at-risk youth.
  • Gathering data and qualitative measures to identify opportunities regarding program initiatives. Using information from visits in accordance with department practices to drive process improvement projects within the program.
  • Collecting and managing data for quarterly reports for grant-funded activities.
  • Meeting and maintaining working relationships with nine other Healthy in the City Community health centers through cross-collaboration with programs and resources.
  • Attending city-wide conferences and workshops provided for Healthy in the City programs.
  • Monitoring access issues relative to insurance coverage, clinical cancellations, and rescheduling.

To qualify, you must have:

  • Associate's degree required. Bachelor's degree preferred.
  • Preferred area of study in Health Management, Childhood Development, Educational Development or a closely related field.
  • Two years of experience working with children or adolescents in a health care, community, or educational setting required.
  • Knowledge of the cultural, social, and economic background of the population being served.
  • Ability to provide counseling services to at risk youth and young adults.
  • Knowledge of case management principles, skills, and methods of relating to the clients is required.
  • Proactive, self-initiating style of work, both in direct patient care and in collaboration with health center staff.
  • Spanish language skills preferred.
