Job Title: Community Health Worker

Duration: 6+ months

Working Hours: Day

Location: Hampton VA 23661

Job Description:

  • Community Health Worker Member of the Population Health multidisciplinary community engagement team.
  • Plays an integral role in educating the public, especially in communities where there is an identified need and information gaps related to heart health.
  • Engages and establishes relationships with local community members and leaders to identify opportunities for outreach and education to build trust.


Role & Responsibilities:

  • Responsible for targeted outreach to underserved or harder-to-reach communities, as well as outreach to local partners and businesses including schools, churches, and stores.
  • Responsible for establishing learning collaboratives (LC) on the Peninsula.
  • Performs a variety of outreach activities including door-to-door flier distribution, meetings with residents and partners, listening sessions and workgroups, health fairs, and community events.
  • Promotes key heart disease messaging in the community and at events which focus on individual residents and families
  • Documents all community outreach activity in online database or spreadsheet.


Community Outreach:

Health Resources Navigation:

  • Works with community members and partners to identify and register individuals who are interested in receiving support via the Unite Us portal.
  • Connects individuals with transportation to doctor appointments and screenings
  • Follows up with individuals, when necessary, by phone calls, clinic visits, email or text and letters.
  • Connects individuals to other needed resources (e.g., food, housing) when appropriate
  • Documents all client interactions in an online database or spreadsheet


Community Advocacy:

  • Assists in sharing community feedback information to local health district leadership team to inform communication, marketing, and event planning.
  • Serves as a community connection point for convening community residents around public health programs, public housing redevelopment, research interventions, and community assessments.
  • Advocates for community change through participation in City and Statewide initiatives
  • Participates in community meetings held by local organizations to ensure resident input.
  • Serves as point person for the Peninsula Community Hub and is responsible for member recruitment, uploading information, facilitating meetings and trainings, and posting relevant information as it pertains to heart disease.


Team and Professional Development:

  • Coordinates activities with supervisor to assure optimum use of time and effort.

  • Participates in in-services, workshops, and classes to maintain knowledge, skills and abilities (KSAs). Attends program-required training/educational sessions.
  • Identifies and requests attendance at appropriate educational programs to enhance job performance.


Preferred requirements/KSAs

  • Knowledge of the geography and jurisdiction boundaries on the Virginia Peninsula
  • Knowledge of organizations, agencies and advocacy groups on the Virginia Peninsula that can connect him/her to groups and individuals.
  • The ability to communicate information clearly and simply.
  • Experience working with diverse populations.
  • Previous Community Health Worker experience.
  • Experience facilitating meetings with community members and interprofessional partners.
  • Knowledge of (or ability to learn) heart health, stroke, and health inequities as community health issues.
  • Knowledge and ability to speak a second language a plus.
