POLICE CO-RESPONDER CASE MANAGER

EMERGENCY SERVICES PROGRAM

FULL TIME - NON EXEMPT

General Summary:Provides direct care, support, education and advocacy to clients, their family members and caregivers. Identifies clients' needs and barriers affecting their mental health, physical health and wellbeing. Partners with clients to assess treatment needs, collaborate on developing person-centered treatment goals and assist with linkage to appropriate referrals. This position is community and office based at the Warwick Police Department.

Schedule: Mon-Fri, First Shift

Essential Responsibilities:




  • Provide screening, education, and follow-up services to clients on issues concerning mental health, physical health, substance use, vocational needs, and living skills.
  • Respond to behavioral health related calls for service with police and provide on scene behavioral health screening, problem identification, crisis intervention, and crisis care coordination and referrals, which are consistent with the identified needs of the client.
  • Identify when an emergency assessment is needed and triage to Emergency Services Clinician.
  • Conduct mental status, suicide risk and needs screenings to identify problems and appropriate interventions, referrals and linkage to resources.
  • Follow direction from the partnering police department regarding on the scene safety, security measures and protocols.
  • Conduct follow-ups and interventions with community members via phone calls, messages, and on-site presence, working to connect clients to behavioral health care and other support services in the community.
  • Collaborate with other providers, agencies, and individuals in the client's network of care


  • Educate clients, their family members and caregivers on mental illness and/or substance abuse disorder as appropriate.
  • Educate clients on effective techniques for recognizing and managing symptoms.
  • May participate in public presentations and engage with community partners and organizations regarding Mental Health Co-Responder Program.
  • Assist in providing ongoing training and education for officers on de-escalation, mental health and substance abuse interventions.
  • Assist clients in developing and strengthening their support network to accomplish goals and objectives.
  • Enters all necessary information in the client record such as progress notes, billing information and data in a timely manner and in accordance with Thrive standards.
  • Participate in clinical team meetings to ensure smooth coordination of client services and treatment.
  • Accompany client to community appointments, provide advocacy as needed.
  • Carry and enliven a positive Thrive image consistent with the Thrive's Guiding Principles and professional code of ethics.

Qualifications

Education, Experience and Competencies:




  • Bachelor's Degree in the Human Service field or related field required.
  • Minimum three (3) years of experience in the Human Service field working with clients who have mental health and substance disorders.
  • Training and experience in Motivational Interviewing, crisis intervention, and Trauma Informed Care preferred.
  • Experience working in collaboration with law enforcement and criminal justice officials preferred.
  • Experience or training in related state and federal assistance programs preferred.
  • Basic computer literacy skills and familiarity with an electronic health record are necessary.



Certifications, Licenses Requirements:




  • Valid driver's license and automobile insurance which meets Rhode Island minimum standards required.

All employees are considered benefit eligible if their schedule is 17.5 hours per week or more. Time-off balances are pro-rated for part-time employees. Insurance plans remain the same regardless of FT or PT benefit eligibility.
