Lutheran Social Services of Illinois is a not-for-profit social service agency with more than 70 program sites throughout the state of Illinois. Our services include counseling, mental health, substance abuse treatment, housing, work with prisoners and their families and services for older adults and a variety of services for children including preschool, foster care, and adoption.
The full-timeCare Coordinator Supervisor will lead LSSI's CCBHC Care Coordination Team and to ensure that quality care coordination is provided to LSSI's Portage Cragin Counseling Center clients. Manage the performance of the team in order to meet key performance measures, improve client outcomes, and meet the CCBHC standards.
Schedule: Monday through Friday 9am to 5pm

Essential Functions:
  • Provide leadership and oversight of the CCBHC Care Coordination Team including hiring, mentoring, training, coaching, and termination.
  • Work with Program Development and other key stakeholders to develop and implement the CCBHC Care Coordination Team.
  • Ongoingly, ensure that the fidelity of the team and care coordination provided meets the CCBHC standards.
  • Ensure that care coordination plans meet the needs of the clients, that care is coordinated across LSSI's continuums, and designated collaborating organizations (DCOs) are incorporated.
  • Manage caseloads to effectively meet the volume of clients referred for care coordination.
  • Track the performance measures of the team. Review and discuss them with team for continuous quality improvement.
  • Ensure that data and documentation are entered by team into CareManager is accurate and timely.
  • Work with Program Development Team to meet the CCBHC grant requirements, perform the reporting, and implement changes when necessary.
  • Participate as a member of the CCBHC Continuous Quality Improvement Team and collaborate for stronger client outcomes.
  • Performs other related tasks as needed.
Position Qualifications:
  • Bachelor degree in nursing.
  • Graduate degree in social work, counseling, psychology.
  • If a Nurse, an unrestricted Illinois license as a Registered Nurse.
  • If a Social Worker, be licensed as a LCSW.
  • If a Counselor, be licensed as a LCPC.
  • 3-5 years' supervisory experience leading a team.
  • 2-3 years' experience in providing care coordination to a behavioral health population.
  • Experience using a care coordination platform, like CareManager.
  • Excellent communication, organization, presentation and pc/computer skills (including proficiency with Microsoft Office - Outlook, Word, Excel and PowerPoint) along with other related software.
  • A passion for behavioral health and inspired by mission-driven work.
  • Driving for home visits and internal/external meetings. Valid state driver's license in good standing is required.
  • Travel in Chicago and surrounding suburbs, as required.
  • Valid Illinois Driver's License in good standing and access to an insured vehicle are required.
