Minimum to Midpoint Pay Rate:$35.04 - $47.30 / hour

Summary

Responsible for providing team leadership for the assessment, triage, care coordination and coordination of the disease education to improve health outcomes for specific moderate to high risk based patient population and wellness activities for low risk patients. Works collaboratively with high risk, value based patient populations. Connects with the patients in person (through home visits), on the phone, on-site in the physician offices, hospital, SNF, and other sites of care. Provides face to-face interaction with patients and their care team when appropriate to improve patient care in person or via telehealth or video visits as needed. This position will have a hybrid schedule.
