Role: Peer E6 Case Manager With the support and in partnership with the E6 team, the E6 Peer Case Manager will support the E6 team successfully engage with people currently living on the streets, for the purpose of getting them matched to permanent, affordable housing. Peer Case Managers will walk the streets of Metro LA (Service Area 4) in teams to identify, engage and assist in housing homeless residents. These teams are part of a new large-scale street-based engagement effort in the Metro Los Angeles Area (Service Area 4). This is a LA County approved strategy to combat homelessness. The goal of E6 is to create a countywide network of multidisciplinary, integrated street-based teams to identify, engage, and connect, or re-connect homeless individuals to interim and/or permanent housing and supportive services.
