Case Manager III We are seeking a Case Manager III to work on our Dual Case Management team to provide the highest level of case management and care coordination through plan development, monitoring, evaluation, and advocacy. Principle areas of responsibility include acting as lead for the program, provide training to other case management staff, and maintain referral source relationships. This position will also support other members of the team in achieving these objectives.
