Description

Case Manager I

People Incorporated is seeking a Case Manager I to join our Targeted Case Management Dakota County team to work with clients who are diagnosed with chronic mental illness. We support people in connecting with resources to live as independently as possible. We help them develop self-awareness to make positive plans for the future through frequent meetings in the community. Case Managers develop long term relationships with their clients and operate autonomously, creating their own schedules.

Hours: Full - Time, Monday - Friday, business hours

Location: Dakota County, Minnesota. Hybrid

Range: $22.00 - $23.50 Hourly, based on qualifications

Hiring Bonus: $1,000 to external candidates

Main Job Duties:

  • Must be able to have onsite presence within the Twin Cities, Minnesota.
  • Perform functional assessments that lead to individual plan development and result in the client receiving access to necessary mental health and related services.
  • Conduct regular review of plan effectiveness and make adjustments as needed that result in the client receiving the maximum amount of assistance and support while monitoring the effectiveness of services received.
  • Assessing client needs and ensuring continuity of care.
  • Making ongoing referrals to resources based on assessment and current client need.
  • Maintaining necessary daily records for continuity of service and reports to funding agencies.
  • Excels at documenting all interventions, interactions and relevant information to the treatment plan consistent with professional and organizational standards.
  • Attending client monthly meetings to monitor services.

Qualifications:

  • Pass all background checks and requirements for the facility and position.
  • Pass the screening required by Federal Human Services Office of Inspector General to assure that person has not been excluded from working in the Health Care Programs which receive federal funding.
  • Must have a valid and current driver's license and auto insurance.

Education and Experience Required for Dakota County Targeted Case Manager I position:

  • Bachelors Degree in the behavioral sciences or related field (including but not limited to social work, psychology, or nursing from an accredited college or university).
  • If no Bachelors Degree in a behavioral science or related field, the employee must meet one of the following:
  • Have 3 or 4 years experience as a case manager aide; or
  • Be an RN without a Bachelors Degree and have 3 years or more combined specialized training in psychiatry and work experience consisting of community
