Job Overview

In response to collected data, implements, and evaluates nursing care designed to meet the individual health and illness needs of complex patients and their families in collaboration with other disciplines. Carries out patient responsibilities under direct supervision of registered nurse with emphasis on family-centered care.

Qualifications

Education: High School Diploma or GED and completion of a Practical Nursing certification or diploma program
Experience: One (1) year of LPN experience.
Certification(s): BLS from the American Heart Association / CPR
Licensure(s): State of Colorado Department of Regulatory Agencies (DORA) Licensed Practical Nurse (LPN)

Responsibilities

POPULATION SPECIFIC CARE

Neonate - <30 days
Infancy - >30 days to 1yr
Toddlers - >1yr to 3yrs
Pre-Schoolers - >3yrs to 5yrs
School age - >5yrs to 13yrs
Adolescent - >13yrs to 18yrs
Adult - >18yrs to 65yrs

ESSENTIAL FUNCTIONS
An employee in this position may be called upon to do any or all of the following essential functions. These examples do not include all of the functions which the employee may be expected to perform.

1. Actively participates in the reinforcement of patient/family education.
2. Contributes to the assessment of the patient's health status, report and record objective and subjective data to assist with carrying out the plan of care and escalates abnormal changes in each body system to RN or Provider for full assessment.
3. Collects, reports, and records objective/ subjective data. Reports signs and symptoms of deviation from expected health or plan of care to RN or Provider for full assessment.
4. Provides routine and common education to patients and families
5. Under the supervision of the RN, provides developmentally appropriate care, interventions and evaluates the effectiveness of interventions and communicate outcomes of care given.
6. Develops goals to advance clinical practice and professional growth through an understanding of the need for self-directed personal and unit based professional and educational activities.
7. Participates in quality and process improvement initiatives.
8. Understands the roles within the multidisciplinary team providing family centered patient care while maintaining objective and professional boundaries.

Salary Information

Annual Salary Range (Based on 40 hours worked per week): $48,962.61 to $73,443.91
Hourly Salary Range: $23.54 to $35.31
