The Walgreens Program is an innovative program dedicated to increasing access to quality healthcare for historically marginalized communities in the Greater Boston area. The focus of the program is to bring the care to where the community can best access it. The Nurse Practitioner is an integral member of the team bringing these health care services which include, cardiometabolic screening/treatment, mental health evaluations, referral management for specialist follow-up (if needed), and patient education on condition management. This position allows the NP to meet the patient, where they are at, in their local environment to best directly assist them with their health care needs. The NP will partner with the Walgreens Pharmacist for prescription management and vaccination access.
