Manager, Case Management

Full-time, Days

Job Requirements:

• The Case Management Manager helps facilitate the management of the case management and admission coordination departments in collaboration with Department Director.
• The Manager oversees a staff of RN's and social workers responsible for patient care coordination and patient flow.
• Implements the developed case management programs including utilization review and intake or discharge planning.
• The Manager helps facilitate the management of the area through the direction of human and physical resources adjusting the daily operations to meet the changing needs/environment of the departments.
• The Manager strives for excellence in patient care and patient care delivery by building a collaborative work environment among all staff and departments.
• The Manager supports an environment of healing and wellness and is an exemplar for compassion, acceptance, respect, and empathy. May cover cases as needed.

Job Requirements:

• Bachelor's degree in nursing
• Ky RN license
• Three years' experience in acute care case management
• Must obtain ANCC, CCM, ACM-RN within 2 years

Preferred Requirements:

• Master's degree in nursing
• Case management certification (ANCC, CCM, ACM-RN)
