Posting Summary |
Rutgers, The State University of New Jersey, is seeking a Program Coordinator for the department of Medicine at the Robert Wood Johnson Medical School.
Under the Direction of the Program Manager, NJ Regional Hemophilia Treatment Center, provides professional social work treatment and discharge planning services to patients and their families related to their health care and general well-being. Provides supportive counseling and/or crisis intervention to patients and families. Formulates educational plans for individuals, families, the Hemophilia Treatment Center and the NJ Regional Hemophilia Program. Communicates to team member's psychosocial factors and family dynamics that may influence the learning process of the individual and family. Facilitates the development and implementation of multidisciplinary projects.
Among the key duties of this position are the following:
- Coordinates social services aspects of the NJ Regional Hemophilia Treatment Center.
- Assesses patients' and families' psychosocial profile and functioning and identifies areas of need for education, support, further in-depth evaluation, or professional intervention and/or therapy.
- Actively participates in the education of patients and their families regarding their hemophilia or other bleeding disorder.
- Identifies and is familiar with social, vocational, rehabilitation, educational, and financial aid resources of State and individual communities of residence of the patients with hemophilia and other bleeding disorders.
- Facilitates resolution of patients' social, financial, vocational, and educational problems through the use of hospital, community, and state resources.
- Responsible for follow-up, coordination, and cooperative efforts with referral resources.
- Provides intervention to hospitalized NJ Regional Hemophilia Program patients whether directly or via collaboration with other professional staff.
- Maintains active contact and coordinates efforts with the social workers of the other Hemophilia Treatment Centers of the NJ Regional Hemophilia Program and the Hemophilia Association of New Jersey.

Qualifications
Minimum Education and Experience |
- Master's Degree in Social Work (MSW) from an accredited professional school.
- At least three (3) years of social work experience in a medical setting required.

Certifications/Licenses |
- Licensed by the State of New Jersey.

Required Knowledge, Skills, and Abilities |
- Facility with electronic medical records and Microsoft Excel.

Preferred Qualifications |
- Familiarity with patients with hemophilia and related bleeding disorders and the associated psychosocial issues.
