Responsibilities The Care Management Associate (CMA) functions as a patient advocate by collaborating with the multidisciplinary care team to facilitate timely and safe transitions of care. The CMA works closely with the care management team as well as the medical and nursing staff to facilitate the coordination of post-acute services. Qualifications No experience required.
