The Role:

As the Director of Care Management at our esteemed Medical Center, you'll be at the helm of the care management functions, orchestrating seamless operations and cultivating a culture of continuous improvement. Your leadership will be instrumental in driving personnel and fiscal management for our Case Management and Social Services departments. You'll design and implement staff development initiatives that align with our strategic goals and uphold professional standards.

What You'll Do:



  • Healthcare Visionary: Lead the charge in optimizing resource utilization and care management across the continuum. Be at the forefront of denial management, discharge planning, and length of stay initiatives, in accordance with the Medical Center Utilization Plan.
  • Strategic Collaborator: Partner with physicians, the management team, and staff to analyze practices and integrate industry best practices, propelling Queen of the Valley Medical Center toward its strategic objectives.
  • Compliance Connoisseur: Ensure our operations are in sync with Joint Commission standards, state and federal regulatory criteria, and professional practice standards.
  • Patient-Centric Leader: Address the age-specific needs of patients from infants to geriatrics, guiding your team in delivering compassionate and informed care throughout their journey.
  • Operational Maestro: Direct and guide strategic program components, including utilization review, clinical documentation management, performance improvement projects, and medical staff relations.
  • Community Connector: Collaborate with our Community Outreach/Care Network program to maintain continuity of care, building long-lasting partnerships with community entities and organizations.
  • Leadership Developer: Mentor, recruit, and cultivate talent within the Case Management and Social Work Departments. Encourage continuous learning and professional growth, enhancing the atmosphere for sacred encounters.
  • Fiscal Advocate: Demonstrate fiscal accountability, develop and monitor budgets, and champion resources for innovative department goals, including new technologies.


What You'll Bring:



  • Educational Excellence: A Bachelor's Degree in Nursing or Health Services Administration is required, with opportunities for current employees or contractors to attain the requisite education.
  • Advanced Expertise: A Master's Degree in Nursing, Social Work, or Health Services Administration is preferred, alongside 5 years of care management experience in an acute care setting.
  • Proven Leadership: With 3 years of leadership experience in an acute care environment, you'll bring a seasoned perspective to strategic decision-making.
  • Certification Advantage: Licensed Clinical Social Worker (LCSW) if possessing an MSW. Registered Nurse if possessing a Bachelor's Degree in Nursing.
  • Communication Champion: Exhibit effective interpersonal, verbal, and written communication skills to resolve conflicts, facilitate collaboration, mentor staff, and convey complex concepts clearly.
  • Innovative Problem Solver: Be proactive in addressing system outcome issues, demonstrating flexibility to adapt and grow within an evolving organization.
  • Technological Proficiency: Bring advanced computer skills and knowledge of software programs, database management, and regulatory requirements.
