GENERAL SUMMARY:

Provides nursing and medical services to assigned patients according to the Scope of Practice for Nurse Practitioners as identified by the American Academy of Nurse Practitioners OR as a Physician Assistant provides primary health care and performs selective medical services under the direction of the appropriate medical doctor(s). Responsible for health care for families and individuals, promoting good health and disease prevention.

PRINCIPAL JOB FUNCTIONS:

1. *Commits to the mission, vision, beliefs and consistently demonstrates our core values.

2. *Conducts patient assessments including obtaining a relevant health and medical history; performs a physical examination based on age and history; performs or orders preventive and diagnostic procedures based on the patient's age and history and identifies health and medical risk factors.

3. *Diagnoses patients by synthesizing and analyzing the collected data; formulates a differential diagnosis based on the history, physical examination, and diagnostic test results.

4. *Identifies and implements a patient treatment plan which may include ordering additional diagnostic tests; selects appropriate pharmacologic and nonpharmacologic interventions; develops a patient education plan; makes appropriate treatment referrals.

5. *Responsible for electronic medical record entries which reflect the patient's healthcare within the allotted timeframe.

6. *Implements patient care plans based on scientific principles, theoretical knowledge, and clinical expertise; accurately conducts and interprets diagnostic tests; accurately prescribes pharmacologic agents and nonpharmacologic therapies; provides relevant patient education; makes referrals to other health professionals and community agencies as needed.

7. *Determines the effectiveness of patient treatment plans; documents patient care outcomes; reassesses and modifies treatment plans as necessary to achieve health and medical goals.

8. Participates as a team member in the provision of health and medical care; interacts and collaborates with professional colleagues to provide comprehensive care.

9. *Accurately documents patient findings, treatment plans and evaluations; documents interventions appropriately to ensure coding compliance for reimbursement purposes.

10. *Provides monitoring and continuity of care between physician visits.

11. Assists with activities related to education, site visits, etc.

12. *Complies with CLIA and OSHA clinical safety practices as well as administrative and facility regulations.

13. Maintains professional growth and development through seminars, workshops, and professional affiliations to keep abreast of latest trends in field of expertise.

14. Participates in meetings, committees and department projects as assigned.

15. Performs other related projects and duties as assigned.

EDUCATION AND EXPERIENCE:

Current Registered Nurse licensure from the State of Nebraska or approved compact state of residence as defined by the Nebraska Nurse Practice Act required. Master of Science degree in Nursing with current Advanced Practice Registered Nurse licensure from the State of Nebraska required. Certification with the U.S. Drug Enforcement Administration for prescriptive authority required within two (2) months of hire. Certification as Adult Nurse Practitioner or Family Nurse Practitioner preferred. Prior adult or family practice experience preferred. Completion of the Bryan Physician Network credentialing and re-credentialing process required for employment.

OR

Graduate degree from a Physician's Assistant program required. Current licensure as a Physician Assistant required. Completion of Bryan credentialing and re-credentialing process required for employment. Certification with the U.S. Drug Enforcement Administration for prescriptive authority required within two (2) months of hire. Prior adult or family practice experience preferred. Completion of the Bryan Physician Network credentialing and re-credentialing process required for employment.

OTHER CREDENTIALS / CERTIFICATIONS:

Basic Life Support (CPR) certification required. Bryan Health recognizes American Heart Association (for healthcare professionals), American Red Cross (for healthcare professionals) and the Military Training Network.
