Description

The Team Leader, Correctional Facility is a part of the management structure of the program. The Team Leader is responsible for the direct supervision of the team to ensure quality service delivery to a special population of patients at the county jail, functioning as the senior clinician or "player coach" of the team. This position reports to the Area Director, Clinical Operations.

ESSENTIAL DUTIES & RESPONSIBILITIES



  • Assumes the leadership role to a therapist and in the daily monitoring and implementation of services. Ensures services are provided to a special population of patients at the McHenry County Jail while they await placement at the state hospital for fitness restoration.
  • Reviews the referrals to the team provided by the Department of Mental Health (DMH). Provides ongoing assessment, makes recommendations for treatment in collaboration with Thresholds prescriber, and coordinates with multiple partners regarding each patient's clinical needs while in the program.
  • Actively leads and oversees the activities of the staff (therapist) provides care coordination, case management, social education, physical well-being, programming, and support.
  • Collects all data and outcomes required by DMH and completes required quarterly reports in a timely manner.
  • Monitors program referrals and coordination of discharges to ensure case assignments, transitions and closures are handled effectively and efficiently with multiple stakeholders such as DMH, Corrections staff, Thresholds program leadership, and the contracted on-site medical team.
  • Completes and maintains all required agency and court documentation. Maintains the quality, validity and confidentiality of individual case records. Assures that all assigned documentation is completed accurately, on time and is organized in a systematic manner.
  • Assists with special tasks as assigned, including but not limited to representing the agency at meetings, orienting and training new staff.
  • Conducts annual evaluations in a timely manner for all team staff. Develops specific goals and objectives and provides the method and opportunity for each team staff to succeed in their professional development.
  • Participates in the hiring sequence of the agency and makes recommendations on the hiring or termination of team staff.
  • Follows agency standards regarding clinical supervision.
  • Other duties as assigned.
  • Schedule: Monday-Friday 8:30am-5:00pm


MARGINAL DUTIES & RESPONSIBILITIES



  • May be required to work flexible schedules and be available to work on weekends or evenings as required.


EDUCATION



  • Master's degree in social work, counseling, psychology or closely related field is required.


EXPERIENCE



  • Two (2) years clinical experience.
  • LCSW or LCPC preferred.
  • Leadership experience preferred.
  • Computer proficiency, including the ability to use email, navigate and enter notes into electronic medical records and working knowledge of pertinent software.
  • Strong interpersonal skills.


SKILLS/CERTIFICATIONS



  • Reliable transportation to correctional facility required.
