Home Health Referral and Admissions Coordinator - Decatur

The Home Health Referral and Admissions Coordinator is responsible for managing the referral process for home health services. It is the responsibility of this position to process all incoming referrals and ensure appropriate documentation including maintaining patient demographic and current insurance information. The Home Health Referral and Admissions Coordinator works closely with the home health team and referring providers to ensure adequate coordination and transition for patients with home health services

Principal Responsibilities



  • Maintain ongoing accurate documentation on referrals, which may require an IT database.
  • Ensure complete and accurate resignation to include patient demographics and up-to-date insurance information. Maintain a patient chart that is up to date with information on specialist consults, hospitalizations, ER visits and community organization related to their care.
  • Gather information concerning a patient's clinical background and referral needs and provide the appropriate clinical information to home health clinicians based on referral guidelines. Ensure referrals are addressed in a timely manner.
  • Contact review organizations and insurance companies to ensure prior authorizations and approvals are met, which will include providing medical history, diagnosis and prognosis as well as providing specific medical information to maximize reimbursement to referring providers.
  • Communicate and review details and expectations with patients regarding their referral to include assisting patients in problem solving any issues that may arise such as financial and social barriers to care and reminding patients of their appointments. Help patients navigate the home health system and advocate for patients and families.
  • Identify and utilize cultural and community resources and establish and maintain relationships with these service providers
  • Comply with the Springfield Clinic incident reporting policy and procedures.
  • Adhere to all OSHA and Springfield Clinic training & accomplishments as required per policy.
  • Provide excellent customer service and adhere to Springfield Clinic's Code of Conduct and Ethics Standards.
  • Perform other job duties as assigned.


Education/Experience



  • High School graduate or GED required.


Knowledge, Skills and Abilities



  • Prior Home Health experience is preferred.
  • Ability to control emotions and maintain composure under stress, using tact and good judgment.
  • Excellent organizational and interpersonal communication skills.
  • Excellent computer and telephone skills.
  • Self-motivated and able to problem solve.
  • Clear, articulate speaking voice.
  • The employee must be able to lift, walk, stoop and sit at a computer for long periods.
  • Must possess manual dexterity to use keyboard and handle equipment and appliances.


Working Environment



  • This job operates in a professional office environment. Routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines.
