Description MaineHealth CorporateSummary: The Case Manager - Medical Outreach role is responsible for the provision of case management services and achievement of positive intervention outcomes for assigned clients. They work on-going with patient caseloads, delving deeper into more medically and socially challenging issues and health needs, addressing barriers for patients. Knowledge Skills And Abilities Responsibilities: Coordinates a comprehensive intake assessment to determine eligibility for case management services to develop plan of care. Education: Bachelor's Degree preferred, with a specialization in Psychology, Behavioral Health, Social Work, Special Education, Counseling, Rehabilitation, Nursing, or closely related field preferred. License/Certifications: Current applicable state(s) license as a Licensed Social Worker preferred. Valid driver's license and reliable vehicle required. Experience: With a bachelor's degree: Two years of experience in a community health setting, including one year of case management experience required. In lieu of bachelor's degree: Three years of directly relevant experience in a healthcare setting, including 6 months of case management experience. Experience collaborating with state and federal agencies, community agencies, municipal, and public programs preferred. Additional Skills/Requirements Required: N/A Additional Skills/Requirements Preferred: N/A * Provides case management in the areas of medical and behavioral healthcare and refers clients to appropriate community-based services when needed. * Documents client interactions, referrals made, and other pertinent client information into the appropriate systems and databases. * Provides client/family education such as clients rights &responsibilities, community resources, and support for family members when appropriate. * Develops and maintains good working relationships with health care providers, community-based agencies, and other departments/disciplines. * Provides education and technical support about the program/department to potential and existing service providers. * Coordinates care across the system through timely communication, advocacy, and accompaniment to client appointments.
