ANNUAL SALARY (DEPENDING ON SKILLS/EXPERIENCE): $132,704.00 - $145,974.40

We are looking for a Nurse Practitioner/Physician Assistant to work with our Endocrinologists in our San Leandro and Dublin clinic locations. Successful candidate would have a sound clinical base of general medicine knowledge, is energetic, adaptable, collaborative and a team player.

The Nurse Practitioner shall be responsible to provide follow-up care to patients with diabetes and endocrine disorders with attention to medication management for glucose, blood pressure, and cholesterol control; other preventive care for diabetic patients; and communication with referring clinicians. In addition, the NP/PA will develop expertise in the management of patients with diabetes and other endocrine disorders and manages a caseload of patients in partnership with the healthcare team to optimize health status and coordinate care over time and across healthcare settings.

Requirements:

• Masters of Science Degree, Graduate of accredited Nurse Practitioner program. Specialty certification required.
• Baccalaureate in Nursing
• Current CA licensure.
