Summary:

The OP Pharmacist reviews, evaluates and verifies prescriptions for appropriateness of dose, route, interval, schedule and duration. Corrects significant drug-allergy, drug-drug, drug-disease, or drug-food interactions. Consults with licensed independent practitioners about outpatient prescription issues and documents interventions in medication therapy. Prepares and checks medication and answers drug information questions.

Responsibilities:

  1. Reviews and verifies outpatient prescriptions.
  2. Evaluates prescriptions for appropriateness of dose, route, interval, schedule and duration.
  3. Corrects significant drug-allergy drug-drug, drug-disease, or drug-food interactions.
  4. Consults with licensed independent practitioners and documents interventions in medication therapy.
  5. Ensures prescriptions meet Ohio legal requirements before dispensing.
  6. Ensures prescriptions are typed, counted, labeled and dispensed correctly. Verifies work performed by pharmacy technicians is correct.
  7. Responsible for accuracy of Controlled Substance Inventory.
  8. Provides drug information and patient counseling.
  9. Assists nurses and physicians with prescription issues.
  10. Reconciles all patient insurance issues prior to prescription filling.
  11. Compounds oral liquid solutions/suspensions and topical medication using appropriate standard operating procedures.
  12. Other duties as required.

Other information:

Technical Expertise

1.Experience in pediatrics/hospital pharmacy is preferred, but not required.

2.Proficiency in MS Office [Outlook, Excel, Word] or similar software is preferred.

3.Proficiency in Epic Willow Pharmacy software is preferred.

Education and Experience

1.Education: Bachelor of Science in Pharmacy or Pharm D Degree is required.

2.Licensure: Current License by the Ohio State Board of Pharmacy is required.

3.Certification: Board Certified Pharmacotherapy Specialist (BCPS) or Board Certified Pediatric Pharmacy Specialist (BCPPS) Certification is preferred.

4.A Post Graduate Year 1 Pharmacy (PGY1) Residency is preferred.

5.Years of relevant experience: 2 years is preferred
