Posting Summary
Rutgers, The State University of New Jersey, is seeking a Patient Navigator for the department of Medicine at the Robert Wood Johnson Medical School.

This position provides patients with assistance in access to healthcare, maintenance of comprehensive treatment and case management services.

Among the key duties of this position are the following:

  • Acts as a liaison for patients in the non-invasive cardiology practice, ensuring timely and coordinated access to care, including diagnostic testing, treatment options, and follow-up services.
  • Supports patient access to care by assisting with attendance at evaluations, follow-up visits, consultations, referrals, and necessary documents (e.g., insurance pre-authorization, school & travel letters).
  • Coordinates and monitors patient appointments, communicates with clinical staff regarding patient flow, and addresses any issues that arise. '
  • Handles patient referrals and pre-certification numbers for procedures.
  • Advises patients on the preparation and requirements of necessary forms and paperwork.
  • Collaborates with the cardiology team to identify patients needing assistance, reminding patients of appointments, arranging transportation, and helping non-English speaking patients access translation services.

Qualifications


Minimum Education and Experience

  • Bachelor's degree in Health, Public Health, Social Work, or a relevant field, plus two years of experience in health or community outreach/advocacy.
  • Equivalent education or experience may be substituted for degree requirements.

Certifications/Licenses
Required Knowledge, Skills, and Abilities

  • Computer literacy, including proficiency in Microsoft Office.
  • Excellent written and verbal communication skills; strong organizational and multitasking abilities.
  • Empathetic, compassionate, and resourceful; able to independently manage multiple tasks.


Preferred Qualifications
  • Experience with Amyloid patients.
