Overview

Completes behavioral health and substance use assessments for problem solving courts, as well as helping to coordinate referrals to appropriate programs and levels of care and other case management activities asked by the court. Works with other departments and programs to ensure clients are placed at the appropriate level of care and that the assessment provided has a sufficient amount of information to justify level of care and programming need.


Qualifications

Certification & Licensure Requirements
  • Licensed Professional Counselor (LPC) within 2 years - Illinois Department of Financial and Professional Regulation (IDFPR);
  • Licensed Social Worker (LSW) within 2 years - Illinois Department of Financial and Professional Regulation (IDFPR);
  • Licensed Clinical Social Worker (LCSW) - Illinois Department of Financial and Professional Regulation (IDFPR);
  • Licensed Clinical Professional Counselor (LCPC) within 2 years - Illinois Department of Financial and Professional Regulation (IDFPR);
  • Certified Alcohol and Other Drug Counselor (CADC) within 2 years - Illinois Alcohol and Other Drug Abuse Professional Certification Association (IAODAPCA),

Education: Master's Degree: Counseling; Psychology; Social Work, Related Field

Other Requirements
Advanced reading, writing and oral skills. Ability to respond appropriately to customer/co-worker Interaction with a wide variety of people Maintain confidential information Ability to project a professional, friendly, helpful demeanor. Basic computer knowledge: Uses word processing, spreadsheet, e-mail application, and web browser. Comfortable within Windows OS and learning new applications. Ability to utilize an electronic medical record.

Responsibilities

ENSURES PROBLEM-SOLVING COURT PATIENTS RECEIVE COMPREHENSIVE SERVICES

  • Works with team members across programs, Referral Sources and Other Organizations in the community to coordinate assessments and admissions.
  • Completes initial and updated mental health and substance use assessments.
  • Coordinates with the patient, referent, etc. any needed paperwork, to be reviewed to determine appropriateness for services.
  • Works with departments/programs to ensure clients are placed at the appropriate level of care and that the assessment provided has a sufficient amount of information to justify level of care and admission into a program.
  • Works with other departments within the system related to referrals and access to treatment services.
  • Works with the Problem-solving court on case management activities needed for the court and the clients.
  • May provide transportation to clients to access resources as clinically appropriate

EVALUATES PATIENT, PRIORITIZES CARE ACCORDING TO ACUITY AND DOCUMENTS APPROPRIATELY

  • Receives referrals from the court, obtains appropriate releases, in order to initiate continuity of care through the risk classification and/or admission/treatment process.
  • Provides patient contact and interaction that is based upon inherent dignity, individuality, culture and beliefs of each patient.
  • Patient care is provided in a compassionate, humane, and non-enabling manner.
  • Provides patient care with attention/knowledge of age specific care.
  • Makes appropriate referrals as necessary and facilitates the referral process for patients as appropriate.
  • Ensures all assessments are scheduled and covered in a timely and appropriate manner.
  • Works well with clerical staff & supervisory staff to ensure coverage of evaluation schedule.
  • Provides follow up for no call/no show and patients who disengage in treatment services.
  • Oversees all service inquiry calls and follows up as needed.
  • Oversees data and outcomes related to assessments, conversion rates, and services inquiry calls.
  • Ensures all data is logged appropriately.
  • Meet all regulatory requirements for both SUPR, HFS, and the Department of Mental Health.
  • Attend all Problem-Solving court meetings as scheduled.
  • Participate in required Problem-Solving Court training.

Compensation and Benefits

The compensation range for this position is $23.83per hour - $39.8per hour. This represents a good faith minimum and maximum range for the role at the time of posting by Carle Health. The actual compensation offered a candidate will be dependent on a variety of factors including, but not limited to, the candidate's experience, qualifications, location, training, licenses, shifts worked and compensation model.
