Inpatient Case Manager

The Inpatient Case Manager works in collaboration with the department director to coordinate, supervise, and/or perform operational tasks within specific departments. They also provide social service coordination, discharge planning, education, support, and advocacy services in collaboration with the Clinical Therapist at Inpatient Behavioral Health in addition they provide coordinated community outreach for the program; develops strategies that result in a positive community image and assists the organization in meeting its goals.

Work Schedule:



  • 40 hrs weekly / 1.0 FTE
  • Days, M-F, 40 hours/week



Required Qualifications:



  • Bachelor's degree in social work required



Preferred Qualifications:



  • 1-3 years relevant experience preferred
