Job Summary

The Mgr-Preventative & Chronic Care Management is responsible for the strategic and operational oversight of the Preventative and Chronic Care Management team. This leader ensures seamless coordination and delivery of preventative health services, patient outreach, chronic disease management, and navigation support across ambulatory and community care settings.

This role oversees the development and implementation of workflows, performance metrics, and engagement strategies that improve patient outcomes, reduce unnecessary utilization, and align with value-based care initiatives. The manager serves as the lead for the team, ensuring consistent oversight of enrollment, care planning, patient education, and interdisciplinary collaboration. This role is responsible for daily operations, productivity of staff, problem solving operational issues, revision of departmental policies in collaboration with the Dir-Ambulatory Care Management. Responsible for developing and maintaining integration between departments within the hospital, outpatient clinics and community resources on behalf of value-based care and attributed lives to assure coordinated care across the continuum.

Essential Functions

  • Directs day-to-day supervision, oversite, and direction to the Preventative and Chronic Care Management staff, including scheduling, team performance, goal setting, accountability, and issue resolution.

  • Reviews reports, patient care assignments, population attribution and caseloads to promote optimal productivity and efficiency and monitor staff documentation and performance goals and standards.

  • Leads quality improvement efforts across all programs and develops systems to track and improve key metrics, including patient engagement, outreach volumes, enrollment, and utilization trends.

  • Ensures compliance with documentation standards, organizational policies, and program guidelines across all team members.

  • Provides mentorship and oversight to the supervisor(s) within the team and supports training, onboarding, and competency development for all direct reports. Identifies staffing needs, supports hiring efforts, and manages employee relations in collaboration with Human Resources and leadership.

Supervisory/Management Responsibilities

  • Job has direct and/or indirect supervision of team members that may include final budget authority, hire/termination authority, performance appraisal responsibility and disciplinary authority. Job will be considered a member of management staff at Prisma Health or affiliate and will have direct reports.

Minimum Requirements

  • Education - Bachelor's degree in Nursing, Public Health, Health Education, Health Administration, or related healthcare field.

  • Experience - Two (2) years of experience in nursing, public health, education, wellness or related field. Three (3) years experience preferred.

Required Certifications, Registrations, Licenses

  • Holds a current RN compact/multistate license recognized by the NCSBN Compact State or is licensed to practice as an RN in the state the team member is working.

  • Certified as CHES (Certified Health Education Specialist), Care Management (CCM), Health Coach or Population Health preferred.

Knowledge, Skills and Abilities

  • Working knowledge of chronic disease states (e.g., diabetes, hypertension, CHF, asthma, COPD, hyperlipidemia) required.

  • Strong understanding of care coordination workflows, including referrals, outreach, and patient navigation preferred.

  • Familiarity with social drivers of health, community resource access, and patient engagement principles preferred.

  • Ability to collaborate across departments and with external partners preferred.

  • Knowledge of electronic medical record (EMR) platforms preferred.
