Summary

Dispenses specialty medications in accordance with laws of the Commonwealth of Pennsylvania and to the latest standards of pharmacy practice including those related to accreditation. Participates in performance improvement and quality initiatives to ensure best patient care. Is an active member of the pharmacy's Quality Management Committee. Stays current with specialty pharmacy therapy knowledge and updates clinical protocols as necessary. Maintains and updates patient care plans and clinical assessments within care management platform.

Job Duties

  • Maintains and develops specialty medication clinical protocols in conjunction with other clinical pharmacists, providers, and literature
  • Ensures patient clinical assessments are completed and scheduled at the appropriate cadence for the patient's condition
  • Participates in quality improvement projects as requested by senior clinical staff or pharmacy director.
  • Develops and reviews Care Plans for patients and makes suggestions on how to improve them
  • Is an active participant in the Pharmacy's Quality Management Program and offers suggestions on improvement projects.
  • Assists technical staff and inventory coordinators when issues arise related to patient care.
  • Reviews PSRs and Near Miss reports to determine trends and assist with operational changes to mitigate future events.
  • Develops and maintains Proactive Risk Assessments for medications
  • Assists with pharmacist training in accordance with company policies and procedures

Minimum Qualifications
  • Bachelor's Degree in pharmacy or
  • Doctorate in pharmacy
  • 3 years of post graduate experience in patient care or
  • 1 year the completion of a pharmacy residency program.
  • Must be efficient with basic computer programs such as e-mail, and Microsoft Office programs.
  • RP - Registered Pharmacist - State of Pennsylvania Upon Hire
  • License to Administer Injectables - State of Pennsylvania Upon Hire
  • BCPS - Board Certified Pharmacotherapy Specialist - Board of Pharmacy Specialties Upon Hire or
  • CSP - Certified Specialty Pharmacist - Specialty Pharmacy Certification Board Upon Hire

Preferred Qualifications
  • 5 years of post graduate experience in patient care.

Physical Demands

Lift and carry 25 lbs. frequent sitting/standing, frequent keyboard use, *patient care providers may be required to perform activities specific to their role including kneeling, bending, squatting and performing CPR.
