Schedule Details:This position is a PRN/As Needed position. Hours available weekdays and weekends, including 4-hour, 8-hour, and 12-hour shifts, for both days and PMs. Weekend rotation of 8 hr shifts Sat and Sun every third weekend required. Minimum commitment: 2 shifts per month of availability, but more hours available as desired. Full-time orientation is preferred for the best onboarding experience; however, we would consider part-time orientation. Choose either being in the on-call rotation (2-3 times per month) or working every 3rd weekend

What You Will Do:

As a Visit RN, you'll provide hands-on care to patients in their homes-delivering skilled nursing care and making clinical decisions that directly impact a patient's health and independence. Whether developing or following a plan of care, you'll work collaboratively with case managers, physicians, social workers, and family caregivers to support optimal health outcomes.


This role is ideal for a nurse who is a strong communicator, critical thinker, and teacher-someone who enjoys working autonomously and appreciates the reward of caring for one patient at a time in a meaningful and personal way.



You Will:



  • Deliver individualized nursing care to home health or hospice patients
  • Develop, follow, or revise a patient's plan of care based on ongoing clinical assessments
  • Provide patient education, medication management, and wound care
  • Collaborate closely with case managers, physicians, and interdisciplinary teams
  • Use EPIC to document visits, track progress, and coordinate services
  • Empower patients and families to manage health needs in the home environment



What You'll Need:



  • Associate's Degree in Nursing required; Bachelor's Degree in Nursing preferred
  • Current RN license in Wisconsin or through an eNLC compact state
  • Valid WI driver's license and acceptable driving record
  • BLS certification (or ability to obtain upon hire)
  • Home health or hospice experience preferred
  • Experience with EPIC preferred
  • Strong communication, problem-solving, and time management skills
