Make a difference where compassion meets recovery. As a Licensed Practical Nurse (LPN) at Grady's Crestview Health & Rehabilitation Center, you'll play a vital role in helping patients regain their strength and independence after a hospital stay. In this short-term, post-acute care setting, you'll be part of a dedicated interdisciplinary team supporting patients who aren't ready to return home-but don't need the intensity of hospital care.

Crestview features 26 sub-acute care beds, two therapy gyms, a dining space, and a dedicated floor for short-term rehabilitation. Patients receive therapies such as physical, occupational, and speech therapy, as well as IV, wound, and trach care-giving LPNs the opportunity to grow their skills in a dynamic and team-based environment.

  Deliver age-appropriate, culturally sensitive, and safe nursing care under the supervision of a Registered Nurse and/or Physician

  Participate in the assessment, planning, implementation, and evaluation of care plans for assigned patients

  
  
• 
  

  Educate patients and their families to support recovery and successful transitions back to independent living

  
  
• 
  

  Collaborate with an interdisciplinary team to support patient goals and outcomes

  Supervise and guide unlicensed personnel in providing quality patient care

  Advocate for patients and promote a supportive, respectful, and healing environment

To be considered for this role, candidates must meet the following requirements:

• 
  

  Graduate from an accredited school of nursing as a Licensed Practical Nurse - Required

  
  
• 
  

  Current Georgia LPN License - Required

  
  
• 
  

  Current American Heart Association Basic Life Support (BLS) - Required

  
  
• 
  

  Previous long-term care experience - Strongly Preferred
