Discharge Planner - PRN

Turning Point is currently seeking a PRN Discharge Planner. Under the direction and supervision of the Director of Discharge Planning, the Discharge Planner provides critical support and care coordination the patients of Turning Point Care Center. The Discharge Planner is responsible for assisting and coordinating with the Clinical Services team to assure that patients have excellent and safe discharge planning, as well as providing recommendations for post-discharge support. Discharge Planners help with all duties including meeting with patients, phone correspondence, discharge paperwork, and written communications with a patient's family and/or community resources such as nursing homes, private mental health practitioners, or halfway houses to meet the patient's aftercare needs. This position is ideal for LPNs or someone with a Bachelor's degree in Social Sciences with healthcare experience.

Responsibilities include:

  • Facilitates timely referrals to alternate levels of care and assists patient with completion of applications.
  • Documents discharge planning contacts in progress notes, communicates with therapists and the treatment team about updates on the status of discharge planning.
  • Participates in problem-solving regarding abrupt changes in discharge, coordinates and communicates the discharge plan.
  • Coordinates discharge appointments and communicates to therapist and family if directed the treatment team.
  • Works closely with Clinical Services, Nursing, Transportation, Medical, Admissions, and Utilization Review departments.
  • Assists nursing department with prior authorizations.

On call coverage will be on a rotational basis.

Qualifications

  • LPN licensure or a Bachelor's degree in Social Sciences with healthcare experience required.
  • One (1) year of experience in a medical field, behavioral health experience preferred.
  • Must be able to interact appropriately with all customers (patients, co-workers, community resources) and possess above average oral and written communication skills.
  • Must be proficient with computer software programs and office machines.
  • Great organizational and time management skills are required.
  • Demonstrate flexibility in providing assistance to varied customers.
