The Job The Clinical Pharmacist is responsible for providing comprehensive pharmaceutical care services to patients in collaboration with SCAN-employed physicians and other healthcare providers within the context of SCAN's Care Management Team. This role supports SCAN's mission to keep seniors healthy and independent by improving medication use, optimizing therapeutic outcomes, and enhancing medication adherence in alignment with SCAN Pharmacy Department goals, including support of HEDIS/STAR measures. You Will Work in collaboration with other members of the care management team including social work care managers, RN care managers, community health workers, and care coordinators when they triage concerns about member medication regimens. Review members on care management identified as having polypharmacy, having high-risk medications and/or with barriers to medication adherence and make recommendations for adjustments to be shared with members and their primary care providers. Support post-discharge medication reconciliation efforts and manage chronic diseases including diabetes, COPD, heart failure, asthma, HTN, and behavioral health conditions in conjunction with the member's healthcare team and provide disease management education as needed to improve treatment adherence. Conduct engaging and comprehensive telephonic medication consultations with members and deliver high quality evidence-based therapeutic recommendations to providers. Participate in Inpatient/IDT/ICT team meetings. Assist in educating other members of the care management team on medication management to improve skills in assisting members with medications in the field and over the phone. Assist members with identifying lower cost options when medication cost is a barrier and then communicate recommendations to member's PCP. Assist in identification of members on Care Management who are on high-risk medications or high-risk combinations, then make recommendations to members and PCPs on safer alternatives. Utilize prescription and medical claim data to generate clinical recommendations for medication management. Your Qualifications Bachelor's Degree required. Current & active Pharmacist License required. Graduate or Advanced Degree or equivalent experience preferred. Collaborative Practice Agreement (CPA) eligibility or existing certification required. Pharmacy license in CA required; with ability to obtain in AZ, TX, NV, and NM preferred. Completion of a PGY1 residency program; PGY2 in ambulatory care or equivalent experience preferred. Board Certification preferred (Geriatric, Ambulatory Care, or Pharmacotherapy). Experience in Medicare Advantage and value-based care models preferred. Bilingual Spanish/English preferred. If bilingual, a test will be administered to access proficiency. Advanced Practice preferred.
