Come join our Care Coordination and Clinical Social Work Department as a Comprehensive Care Coordinator at UCLA Health. Under the direct supervision and guidance of the Manager of Care Coordination and Clinical Social Work and the Systems Director for Care Coordination, you will work with an interdisciplinary team of physicians, case managers, social workers, and other care teams at the hospital to identify, assess, and assist homeless patients needing resources. You'll help provide links to medical centers, shelter, recuperative care, pharmacy and other vital resources to our homeless patients at time of discharge and throughout the care continuum. Salary offers are determined based on various factors including, but not limited to, qualifications, experience, and equity. The full salary range for this position is $32.20 - $51.36 Hourly. The budgeted salary range that the University reasonably expects to pay for this position is about $32.20 - $37.00 Hourly. Qualifications
