Job Overview:

The Patient Navigator for the Brain Injury Neurorehabilitation Program provides care coordination for patients and families seeking neurorehabilitation services by serving as an active liaison between patients and families, medical subspecialists, therapies, admissions, insurance companies, schools, community medical providers and social service agencies within a family centered care model.

Qualifications:

Required:

  • Bachelor's degree with experience in medically based care coordination.
  • Excellent communication and organization skills, strong team player.

Preferred:

  • Experience in Pediatrics. Master's degree preferred.
  • Understanding of brain injury preferred.

Essential Functions:

  • Coordinates care for children and teens with brain injury across continuum from intake, outpatient and inpatient services.
  • Coordinates communication between patient and family, internal and external service providers.
  • Demonstrates an understanding of family centered care, patient s medical diagnoses, prognosis, progress and implications for family support, education and goal directed treatments and therapies.
  • Provides triage for families seeking services for brain injury to determine service needs.
  • Reviews and organizes intake materials and consults with brain injury team as needed.
  • Demonstrates an understanding of the health care system, managed care and resource utilization.
  • Ensures scheduling of evaluations for potential patients with brain injury.
  • Facilitates admissions to outpatient Neurorehabilitation program by working with inpatient Patient Care Coordination department for facilitation, coordination and transition planning.
  • Facilitates transition from inpatient to outpatient services.
  • Advocates for the unmet needs of patients and families across the medical, social and financial continuums.
  • Provides necessary documentation to external agencies as needed.
  • Other duties as assigned by supervisor and in collaboration with Brain Injury and Neurorehabilitation program.
