Transitional Care Health Coach

The Transitional Care Health Coach is responsible for providing support, education, and guidance to patients as they transition from hospital to home or between different care settings. This role aims to improve patient outcomes, reduce hospital readmissions, and enhance overall quality of care.
Health coaches are registered nurses located in selected clinics, hospitals and/or home based that provide education, support and self-management tools aimed at improving the member's health.
Typical Hours: 8-4:30pm, M-F

Essential Functions
  • Professional Health Coaching Services for Oklahoma Medicaid Members.
  • Conduct comprehensive assessments and evaluations for individuals with diverse healthcare needs and gathering pertinent medical and social information to facilitate the development of patient-centered action plans.
  • Foster collaborative relationships with healthcare providers and case management teams to ensure coordinated care delivery.
  • Provide evidence-based education and information to members, enhancing their understanding of health conditions and treatment options.
  • Provide education on medication management, symptom recognition, and self-care techniques.
  • Identify and coordinate referrals to appropriate statewide community resources, addressing transitional care and social determinants of health.
  • Coordinate follow-up appointments and ensure patients have necessary resources upon discharge.
  • Conduct hospital visits to monitor the patient's progress.
  • Actively engage patients and their families using motivational interviewing techniques to promote behavior change and improve health outcomes.
  • Teach self-management skills and provide ongoing support to members and their families, empowering them to take an active role in their healthcare journey.
  • Utilize a holistic approach to care, considering physical, emotional, and social factors that impact overall well-being.
  • Maintain accurate and timely documentation of all patient interactions and interventions in compliance with organizational and regulatory standards.
Requirements
  • This role requires a compassionate, patient-centered approach combined with strong communication skills, adaptability and strong-problem solving skills.
  • Registered Nurse (RN) licensed in Oklahoma with an active, unrestricted credential.
  • Extensive experience in disease management and case management, complemented by exceptional communication skills.
  • Demonstrates proficiency in documentation and possesses strong writing abilities.
  • Self-motivated professional with a proven track record of delivering high-quality patient care.
  • Bilingual preferred.
  • Case management certified preferred.
