Job Summary:

The Sepsis Coordinator works in collaboration with the medical and hospital staff to provide guidance on the evidence based care of sepsis patients. The coordinator will provide clinical and professional expertise for patients, families, and staff members. The coordinator will collect and analyze data surrounding the sepsis patient population; driving implementation of evidenced-based practice as related to sepsis care.

Principal Duties and Responsibilities:

The following is a summary of the major functions of this individual's job. He or she may perform other duties, both major and minor, which are not mentioned below, and specific functions may change from time to time.

Evaluation of patient outcomes.

Facilitate evidence based practice across all disciplines in accordance with current sepsis guidelines.

Maintain current knowledge and competency in the management of the septic patient and in professional practice (e.g. conferences / workshops, professional organization, etc.)

Coordinate the Sepsis Program

Communicate effectively and work cooperatively with others using an integrated approach toward achieving optimal patient outcomes.

Role model critical thinking by demonstrating active and proactive problem solving.

Lead PI processes recognizing trends and exploring alternatives.

Develop and implement sepsis related processes reflecting current evidence.

Interpret and present data for key stakeholders.

Identify learning needs of professional staff.

Provide education in order to advance staff and patient knowledge.

Community outreach to provide education on early recognition.

Minimum Education, Training and Experience Required:

Active Kentucky RN license required.

BSN required, MSN preferred.

5 years minimum nursing experience required with experience in Critical Care or ER.

BLS required
