Position Summary/Department Summary:
This 20-hour position covers the Bridges Adult Transition Program's clinical social work needs, including inpatient consultation and virtual visit support for young adult patients and their families (with some limited inpatient Cystic Fibrosis coverage). Social work is an essential part of the interdisciplinary care for a population of young adult patients transitioning to adult care programming. This population has specialized needs and is at particularly high risk for several complex social and medical needs where social work is consultative. Key Responsibilities:
