Job Overview As a Registered or Licensed Practical Nurse, you will be responsible to assess patient needs, develop or assist with care plans and maintain nurse interventions to ensure optimal health outcomes.
LPN Duties: Monitor vital signs regularly-check and record patients' blood pressure, pulse, temperature, and respiration. Administer and dispense prescribed medications under the direction of an RN or physician. Assist with daily activities-help patients with personal hygiene tasks such as bathing, dressing, and grooming. Assist with dressing changes and wound care procedures. Maintain accurate and up-to-date patient records-document patient care activities, update medical charts, and ensure accurate record-keeping. Monitor patients for changes in condition and respond appropriately. Adhere to infection control protocols to prevent the spread of illness. Collaborate with interdisciplinary teams to ensure coordinated patient care. Provide specialized care for the individual patient in their home. Other duties as needed.
RN Duties (in addition to LPN duties): Conduct comprehensive assessments of patients' health status and needs. Implement and evaluate individualized care-based patient care plan and patient assessments. Administer medications and treatments as prescribed by physicians. Provide education to patients, families and staff regarding health management and disease prevention. Assist in diagnostic evaluations and procedures. Other duties as needed
Qualifications: Valid NJ State Registered Nurse (RN) license OR Licensed Practical Nurse (LPN) as applicable. Strong clinical skills with experience in areas such as infection control, diagnostic evaluation. Excellent communication and interpersonal skills for effective collaboration with patients, families, and healthcare teams. Commitment to ongoing professional development and adherence to best practices in nursing care. Other qualifications mandated by the State for In Home Care. Must be able to meet the physical demands of clinical patient care.
