We are seeking a Case Manager - Young Adult Population to join CTI's Youth Services team, located at the Youth Opportunity Center. This Case Manager will provide support to young adults (ages 18-24) entering programming. This includes providing entry level case management, advocacy, referral information and supporting participants in overcoming barriers to long-term, independent housing.

Responsibilities Include:



  • Work closely with individuals to identify and build upon strengths and develop strategies to address barriers and concerns identified through the assessment process
  • Comfortable working in a Residential Setting, supporting with ADL's, and utilizing a trauma-informed approach
  • Assist other Youth Services team members as needed, supporting programming and difficult or complex situations
  • Receiving and monitoring new referrals to the Young Adult Population Programming
  • Developing and using network of available services to assist participants and staff
  • Supporting and completing onboarding and program entry documents with staff and participants
  • Collaborating with Young Adult Population staff to support participants in service-planning
  • Establishing and maintaining working relationships with internal/external partners
  • Data input, file management, case management software and all administrative tasks as necessary


Qualifications:



  • Prior related experience required
  • One year of assessment, advocacy and case management work in a Human Service field strongly preferred
  • High School Diploma or Equivalent required
  • Prior experience working with at-risk youth
  • Ability to work comfortably with at-risk youth in both group and individual settings, preferably with experience supporting at risk youth entering into employment and post-secondary education
  • Demonstrated knowledge of community resources available to low-income individuals and families
  • Excellent customer service, organizational, oral and written communication skills
  • Ability to manage multiple priorities and work in a fast-paced environment
  • Proficient computer skills required in basic computer software and data collection systems
  • Excellent crisis management skills
  • Ability to maintain accurate records and confidentiality of information
  • Excellent attention to detail
  • Bilingual (English/Spanish, English/Khmer, English/Portuguese) preferred
  • Valid Driver's License
